The Journal of Nutrition Supplement: Achieving Optimal Growth in Preterm Infants and Children Intrauterine-Like Growth Rates Can BeAchieved with Premixed Parenteral Nutrition Solution in Preterm Infants

چکیده
Growth failure in neonatal intensive care units is a major challenge for pediatricians and neonatologists. The use of early ‘‘aggressive’’ parenteral nutrition (PN), with >2.5 g/(kg d) of amino acids and at least 40 kcal/(kg d) of energy from the first day of life, has been shown to provide nutritional intakes in the range recommended by international guidelines, reducing nutritional deficit and the incidence of postnatal growth restriction in preterm infants. However, nutritional practices and adherence to recommendationsmay vary in different hospitals. Two ready-to-use (RTU), premixed parenteral solutions (PSs) designed for preterm infants have been prospectively evaluated: a binary RTU premixed PS from our hospital pharmacy and a commercially premixed 3-chamber bag (Baxter Healthcare). These premixed PSs provide nitrogen and energy intakes in the range of the most recent recommendations, reducing or eliminating the early cumulative nutritional deficit in very-lowbirth-weight infants, and avoiding the development of postnatal growth restriction. A further rationale for RTU premixed PSs is that preterm infants require balanced PN that contains not only amino acids and energy but also minerals and electrolytes from the first day of life in order to reduce the incidence of metabolic disorders frequently reported in extremelylow-birth-weight infants during the early weeks of life. The present challenges of parenteral nutrition in preterm infants and children.
The goal of pediatricians involved in the nutritional management of preterm infants is to mimic intrauterine growth and to obtain a functional outcome comparable to that for infants born at term. Appropriate administration of nutrients in the first few days to weeks of life will reduce the growth restriction that is frequently observed. Existing guidelines advise providing preterm infants with ...
متن کاملRegulation of proteolysis and optimal protein accretion in extremely premature newborns.
Growth outcomes for extremely premature infants remain poor, and improving growth in this population will require a better understanding of how to limit proteolysis and promote protein accretion. Extremely premature infants exhibit high rates of proteolysis that are unrestrained by physiologic increases in insulin, intravenous amino acids, and full parenteral nutrition. Imbalances in current am...
متن کاملThe Journal of Nutrition Supplement: Achieving Optimal Growth in Preterm Infants and Children A Systematic Review of Practice Surveys on Parenteral Nutrition for Preterm Infants
Significant efforts have been made to improve the nutritional support of preterm infants in neonatal intensive care units (NICUs) to avoid cumulative nutritional deficits, reduce postnatal growth restriction, and promote optimal long-term development. The objective of this systematic reviewwas to compare the characteristics and results of all surveys published in the past 10 y (2002–2012) that ...
متن کاملThe fear of necrotizing enterocolitis versus achieving optimal growth in preterm infants--an opinion.
UNLABELLED Very-low-birthweight (VLBW) infants suffer marked growth delay despite well-intentioned efforts at combining enteral and parenteral nutrition. Fear of necrotizing enterocolitis (NEC) has traditionally influenced neonatologists toward delaying and progressing slowly with enteral feeding, while supporting the infant with parenteral nutrition. Current evidence suggests significant benef...

Objective: The aim of this study is to evaluate the incidence and risk factors for Intrauterine Growth Retardation (IUGR) and Extra Uterine Growth Retardation (EUGR) and provide more approaches for appropriate nutritional support for preterm infants. Methods: A total of 144 preterm infants (75 male and 69 female) were involved in this study. According to the relationship between gestational age...
